Assess and follow up on patient needs within the palliative care clinic, facilities and patient home environment. Completes consultations for hospitalized patients needing acute palliative symptom management as needed. Works in partnership with the Palliative Care team and IDG to develop and implement the best course to meet the needs of the patient and family. Must be skilled in communication and understand chronic and terminal disease progression and prognosis; must be proficient in knowledge of case to conduct family meetings for goal setting and decision making; collaborates with other healthcare providers and identifies referrals to community resources when appropriate; utilizes telehealth technology. Serves as resource for outpatient palliative care program.
Current NC RN license or compact state RN license. Prefer 2 years' experience in a clinic, home health or hospice. Palliative care or end-of-life care experience preferred. Valid NCDL..
