Patient Care Manger
The position of Patient Care Manager in the new Hilton Head office reports to the Program Director.

Qualifications:
  • Experienced registered nurse (RN) with acute, post-acute and/or long-term nursing care experience, ideally with hospice care experience.
  • Experience caring for patients in a facility and/or home setting.
  • Previous experience in managing other care team individuals for at least two (2) years.
  • Self-starter and able to work independently without direct supervision.
  • Responsibilities and Requirements:
  • To provide leadership, management, and clinical guidance to our Interdisciplinary Teams (IDTs), as well as foster Beacon’s culture of transparency, professionalism and excellence in all aspects of patient care.
  • Serve as the bridge between our care team members, compliance organization, operations, and market-facing professionals.
  • Create a positive and open culture that respects the care team members’ various contributions.
  • Able to sit for long periods of time, lift up to 50 pounds, and also stoop and bend.
  • Duties: The Patient Care Manager (PCM) shall be responsible for the day-to-day management of the care team members, including but not limited to:
  • Coordinate with administrative staff to ensure Beacon continues to remain in compliance with Medicare Conditions of Participation.
  • Interact with Corporate Compliance Nurses to distribute training and monitor compliance efforts.
  • Schedule and facilitate IDT meetings and staff.
  • Work with Community Liaisons to gather information and qualify potential patients using Medicare guidelines and sound clinical judgement.
  • Work with administrative staff, as well as Nurse Case Managers, to manage costs of medications, DME and medical supplies.
  • Employ qualified personnel and ensuring adequate employee orientation, training and in-services, education and evaluation
  • Coordinate branch QAPI activities
  • Ensure the accuracy and completeness of patient records
  • Maintain compliance with applicable laws and regulations and implementing corrective action in response to reports of hospice program committees and regulatory agencies
  • Implement the policies and procedures approved by the governing body
  • Determine staffing needs and staffing patterns according to program requirements and standards
  • Supervise and evaluate Registered Nurses (Case Managers), Aides, Social Workers and Chaplains
