SC House Calls

We understand a person’s home is defined as where they reside. SC House Calls can bridge the gap in care. Whether an acute illness, hospitalization or injury, we will see the patient earlier. Additionally, SC House Calls coordinates care with the patient’s primary care physician. Providers work in collaboration with the skilled facility, assisted living staff to strengthen the practitioner-patient relationship and improve the overall wellness of residents. SC House calls model provides 100 doctors and nurse practitioners to see patients wherever they call HOME. We staff a 24/7 Center for Telehealth with professional medical personnel and deliver home-based palliative and primary care directly to the patient
FT or PT Palliative Nurse Practitioner- NP
Opportunities are available for Palliative Care Nurse Practitioners in the Lowcountry, Beaufort, Pee Dee, Midlands, Grand Strand, and Upstate regions of South Carolina to provide inpatient and/or community based palliative care consults. Currently we have over 125 providers statewide serving all 46 counties, and in order to grow our services, we are interviewing Hospice and Palliative Care Nurse Practitioners for positions across the state. Our goal is to bring inpatient and community-based palliative care services directly to patients, while collaborating with other care providers to maximize quality of life for patients with serious illness.

Qualifications:
  • Skilled in communication, competent in time management, and autonomous in clinical decision making.
  • Current license to practice in the state of South Carolina.
  • Current SC Controlled Substance & DEA.
  • Certification in Hospice & Palliative Care (ACHPN) or commitment to pursue certification.
  • Minimum of 2 years experience as a nurse practitioner with hospice, palliative, geriatric or home care experience.
  • BLS or ACLS Certification

    Position Summary: Lutheran Hospice is currently recruiting for a full-time RN Case Manager for our Midlands Office. With the Interdisciplinary Team, the RN Case Manager builds from the resources of the hospice and community to plan and direct services to meet the needs of individual and families within their homes and communities.

    Position Qualifications: Graduate of an accredited school of nursing and licensed to practice as an RN in SC. Minimum of one year of nursing experience.
