The Toward Accountable Care (TAC) Consortium and Initiative is pleased to announce the release of its latest specialty toolkit for members of the medical community interested in learning more about how their practice fits into the value-based model called an accountable care organization (ACO). The Carolinas Center for Hospice and End of Life Care (TCC) was the only hospice association to contribute to the creation of this informational guide.

The North Carolina Institute of Medicine’s (NCIOM) Task Force on Alzheimer’s Disease and Dementia has  invited The Carolinas Center for Hospice and End of Life Car (TCC) to participate in its year-long efforts to  define a plan for managing Alzheimer’s disease in the state.
